Member Agreement for Discount Medical Plan(s)
Disclaimer: This discount plan is not health insurance. The plan provides discounts at certain health care providers for medical services. The Plan does not make payments directly to providers of medical services. Members are obligated to pay for all health care services at the time the services are performed, however will pay discounted out-of-pocket cost to the providers. This Plan is available in all states in USA. Plan administrators have no liability for providing or guaranteeing service or for the quality of service rendered. Participating providers are subject to change without notice and are not available in all areas.
- Entire Agreement: All provisions under this Agreement, savings card, Application, and product descriptions constitute the entire Agreement between the Company and the Member. This contract is not protected by any state Life and Health Guaranty Association. Discounts on professional services are not available where prohibited by law.
- Complaints: complaint regarding Plan Membership should be directed to Customer Service at the toll-free number on the Membership card, or in writing to info@globalhealthb.com.
- Effective Date and Renewal: Your effective date is indicated on you Savings card. We will not automatically renew your card, except you ask us.
- The Medical savings card is for individual use. You can purchase as many cards as possible for your household with one account.
- Refund: Global healthBrigde will refund any return based on request within 30days of purchase less administrative fee of 3%. Please contact us at info@globalhealthb.com
- Card: Member will be provided with a Membership Card. Such card and other forms of identification should be carried by the Member ID to provide proof of the right to Eligible Services under the Membership Agreement. The discounts contained herein may not be used in conjunction with any other discount plan or Plan.
- By downloading the card, you agree to the terms and condition of this website.
- Agreement: This agreement covers the terms and conditions of your participation (member) in the Global HealthBridge Discount Program when accessing healthcare providers in the United States. Your enrollment in the discount program, subject to payment of the specified premium, begins on the effective date and ends on the termination date shown on your Member Confirmation Letter or Member ID card.
Important Information:
- GLOBAL HEALTHBRIDGE DISCOUNT CARD is not insurance and not a provider of healthcare services.
- GLOBAL HEALTHBRIDGE DISCOUNT CARD does not make any payments on behalf of the member to the providers of healthcare services.
- GLOBAL HEALTHBRIDGE DISCOUNT CARD provides access to the ValuePoint Multiplan network which includes 540,000 practitioners, 1,400 hospitals, and 68,000 ancillary facilities.
- The average savings (discounts) are 39% for practitioners, 17% for hospitals and 33% for ancillaries and 50% on prescription.
- The Member must pay for all healthcare services at the time of service and will receive the appropriate discounted price from providers participating in the ValuePoint network.

Accessing Providers:
Participation in GLOBAL HEALTHBRIDGE DISCOUNT CARD by the Member is voluntary, subject to payment of the specified premium. Selection of providers and services is the sole responsibility of the Member. The Member has the sole authority and responsibility for all healthcare decisions and not GLOBAL HEALTHBRIDGE DISCOUNT CARD.
